We will also have some permission slips at the church when you guys arrive, just in case, so you can have them sign it then if this didn’t work out.

Chick-Fil-A™ Cow Appreciation Day!

Today is Chick-Fil-A™ “Cow Appreciation Day.” Where if you dress up like a cow, you get a free meal! Today we’ll be taking the students over to Chick-Fil-A™ to get a free meal during the part of youth group where we usually have a group game. We have enough costumes for each student. So NO MONEY is required…just your permission for us to take them! Please fill out the following accordingly.

I (your name, please print) _________________________, am considered the legal guardian of (child’s name, please print)__________________________. And I hereby give permission to attend “Cow Appreciation Day”, at Chick-Fil-A™ Restaurant, with Calvary Chapel Quakertown’s Youth Group.

I release and hold harmless Calvary Chapel Quakertown and its youth leaders from any and all actions of any nature of personal injury arising from my child’s participation in this activity.

I/We understand all reasonable safety precautions will be taken at all times by Calvary Chapel Quakertown and its agents during the events and activities.

I, therefore grant permission for first aid to be administered, proper treatment and/or hospitalization to be secured for my child in case of emergency according to the best judgment of the youth leaders and other adults on site.
